Health Plan

Eligibility:

Eligibility commences the first day of the month after you have accumulated 280 hours. Notification of start of benefits is sent to your home by letter

Continuation:

140 hours per month deducted from your hourbank for one month of coverage

Hourbank hours are not limited in your account

Life Insurance:

Member    $50,000.00

Spouse        $10,000.00

Child        $5,000.00

Accidental Death:

Member $50,000.00

Short Term Disability:

Journeyman $350.00 per week

Apprentice $265.00 per week

Two week elimination period, maximum 52 weeks payable including elimination period and integration with EI Benefits

Long Term Disability:

$1,000.00 + $100.00 in RRSP per month after expiry of short term disability benefits until the age of 65

Extended Health:

Drugs paid at 80%

Dental 1 and 2 paid at 80%

Dental 3 paid at 60%

Dental 4 (orthodontics) paid at 50% up to a lifetime maximum of $1,500.00 (children only)

Prescription Glasses/contacts paid at 80% up to a maximum of $250.00 every two calendar years

Eye exam fee once every two years paid at 80%
